Danfoss AF-range stem extension ZF 4, FKM (OEM designation "Stem extension ZF 4, FKM, AF range"). The stem extension is a spacer piece fitted between the valve body and the self-acting AF-range actuator, giving clearance for pipe insulation and separating the actuator from the hot valve body.
